Payment Policies
This program is a comprehensive program that includes a variety of disciplines, client management services, and clinical services. For this reason, our program is billed in advanced on a monthly basis. The program cost includes all clinical visits, administration fees, case management, and treatment team management. The Mindful Health Foundation believes in providing comprehensive care from a team. We do not “nickel and dime” our patients for phone calls, emails, secretarial fees, treatment team evaluations, ect. This is why we have one fee that encompasses all aspects of treatment. The Mindful Health Foundation believes strongly in a team approach and it is a team of practitioners that will be working towards your recovery.
Your monthly billing will occur on a thirty day cycle starting on your date of admission. We accept cash, check, Visa, MasterCard, American Express, and Discover.
Missed Appointment without Adequate Prior Notice
Because we schedule large segments of time for appointments, we must charge for missed appointments. If you must reschedule or cancel an appointment, you must notify us 24 hours in advance to avoid a charge. If you are unable to speak directly with the practitioner, please leave a message with answering service to avoid a charge. Please Note: Charges for missed appointments will be at the full rate.
Finance Charge Policy
Please note that any outstanding balances are subject to a finance charge if not paid at the designated and agreed upon intervals.
Returned Check Policy
Also be aware that returned checks are costly to you and us. Not only will your bank charge you, we will charge you a fee of $25.00 and insist on cash or credit card in the future.
Sliding Scale Policy
In an attempt to provide services to the community, we do offer a sliding scale fee schedule and this is based on household income level. In order to qualify, you must submit 4 weeks paycheck stubs, a year end tax return and two months recent bank statements in writing before entering the program. The Program Costs Our Intensive Outpatient Program costs $3500.00 per month. Clients attend daily. Our Standard Outpatient Program costs $2400 per month. Client's attend bi-weekly. Our After Care Program costs $1300 per month. Clients usually attend once a week. Additional Services may be cahrged seperately and costs are as follows: $125.00 Individual Therapy ( 45 min.) $100.00 Nutrtion Therapy ( 60 min) $50.00 Group Therapy $125.00 Individual Body Image Therapy ( 60 min) $150.00 Family Therapy Sliding Scale fees start at the following household rates: 1 in the household starts at $27,930.00 2 in the household starts at $37,470.00 3 in the household starts at $47,010.00 4 in the household starts at $ 56,550.00 We are very proud of our fee schedule and sliding scale. Intensive Outpatient is a viable alternative to inpatient in some instances. The average cost of inpatient treatment is unfortunately between $30,000.00 and $50,000.00 per month which often makes it financially unfeasible. Insurance companies will often only cover a portion of inpatient treatment. We are often asked if we take insurance. We do not. It is our experience that when a facility has to be the one who negotiates or haggle with the insurance company, it either results in higher costs to the patient because we would have to hire more staff to deal with the insurance company or a distorted treatment provider-patient relationship which decreases the quality of care. We unfortunately receive no outside assistance, donations or grants at this time to supplement treatment.
